Records Request Information

If you are requesting records from my practice, please review the information below carefully before submitting your request.

To protect your privacy and ensure compliance with HIPAA, all records requests are handled through a secure process.

What to Expect

By submitting a records request, you acknowledge and understand that:

  • A signed Release of Information (ROI) is required before any records can be released

  • Former clients must upload a government-issued photo ID to verify identity

  • You will be invited through the secure client portal to complete your ROI and upload your ID

  • Once your identity is verified, records are shared through a time-limited, password-protected secure access link

  • Records are accessed only through the secure portal and are not sent by email, text message, or social media

  • To avoid delays, your ROI must clearly include:

    • The full name of the provider or practice receiving records

    • A secure email address for the recipient

    • Any specific limitations, such as dates of service or document types requested

Incomplete or unclear requests may delay processing.
